Dear CIIS Community,

Last week, I shared the CIIS vaccine requirement and want to follow up with further information.  As a refresher, CIIS is requiring that faculty, staff, and students who enter our two campuses - Mission and Arkansas - be vaccinated and submit medical verification / declination statement to our third-party HIPAA and FERPA compliant tracking service called Med + Proctor. Please check your CIIS email address for a unique email from Med + Proctor where you can upload your documents. The deadline to submit this verification is September 15th. Please note that it takes 72 hours for your verification to appear on the site. Frequently Asked Questions (FAQ’s) regarding the Med + Proctor verification process can be found at the bottom of this email.

Anyone who will not / does not intend to access campus facilities will not be impacted if not vaccinated; however, you will still need to complete the verification or declination form you received from Med + Proctor last week. We need to be able to report vaccination data to various agencies.

In line with our mission and educational philosophy, vaccination against Covid-19 is required because near-universal immunization of a close-knit campus community protects all members: students, staff, and faculty alike.

CIIS also recognizes that in rare instances, individuals’ health conditions or religious commitments may rise to level of a necessary exemption. A person’s personal attitudes, beliefs, or preferences are not grounds for an exemption. To request an exemption for either medical or religious commitment please follow the link below where you will find more information and the necessary forms at the bottom of the webpage, and note the following exemption request deadlines:

  • Deadline to submit your request for vaccine exemption is August 6, 2021.  
  • Deadline to submit additional supporting documentation, as requested by the review panel, is August 13, 2021.

 Med + Proctor FAQ’s:

  1. Do I need to know my Colleague ID to log on? No - you should just need your CIIS/MyMail email address.
  2. Do I need to pay anything? No – the Med + Proctor service is free so ignore any prompts about payment.
  3. Do I need a medical professional to sign anything? No - your vaccine card or other proof of vaccination is the medical verification - that's all you need to upload.
  4. The email is addressed to "Dear Student”, but can faculty or staff upload their documents in same place? Yes - this is meant for entire community. It's just a university service so that's why it says, "Dear Student." Go to student section in portal as well as each member of our community (including faculty and staff) has a unique record there.
  5. When will I see my verification on Med + Proctor page? They told us it would take about 72 hours to show up. That's how long it typically takes for their team to medically verify the document.
  6. Do I need to be able to scan and send a PDF or photo image? Yes.

I want to thank everyone again as we navigate the turbulent waters of the pandemic together. I’m pleased to say that overall our community is 90% fully vaccinated.
