Dear Community,  

As previously stated in other communications, Cal/OSHA regulations require us to track and verify our community’s vaccination status to ensure everyone's safety. Getting vaccinated is still the best way we can protect ourselves and our fellow community members from the COVID virus. Check out this link for more info about getting vaccinated. 

CIIS is now mandating that anyone who enters our two campuses - Mission and Arkansas - be fully vaccinated and submit medical verification to our third-party HIPAA and FERPA compliant tracking service called Med + Proctor. The deadline to submit your vaccine verification is September 15th. 

Please check your CIIS email address (MyMail for students) in the coming days for a unique email from Med + Proctor with a link where you can register your account and upload your documents. Instructions on how to upload your documents are attached.  Please note that it takes 72 hours for your verification to appear on the site. If you don't receive an email from Med + Proctor by Monday July 26th, please email OperationsRequests@ciis.edu. 

If you have a request for a medical or religious exemption, please email the following for further information:

You will still need to register your declination to get vaccinated with Med + Proctor - see attached.

Once verified by Med + Proctor, please request a sticker at the Mission campus lobby front desk to place on your ID card so that you can show that you have been successfully verified when you enter campus.    

In the interim period between now and September 15th, every person entering either campus will need to fill out a Health Screening Form daily, which asks for vaccination status.  

Those not yet fully vaccinated will need to have a negative COVID test within 72 hours of entering campus. Our third-party service Med + Proctor will also be able to track your test results in a private way. Please see the list attached of free vaccine and testing resources near CIIS. If you are having trouble locating a testing site or securing a vaccine appointment, please email OperationsRequests@ciis.edu  

No one is expected to disclose their vaccination or testing status verbally at our front desk unless they choose to, but we are relying on our electronic health screener to check everyone's status daily and keep everyone on campus safe. If the health screener has not been filled out in advance, there is a QR code at front desk where it can be filled out on your phone before entering. It takes about 30 seconds.

FAQs about the Med + Proctor Vaccination Verification Process at CIIS

I never received a registration email from Med+Proctor, what do I do?

You can use this link to register your Med+Proctor account, even if you have not received an email from Med+Proctor. Please note that with this link, you will skip to step #3 on the “Vaccine Card Upload Instructions”. 

Do I need to know my Colleague ID to log on? 

No - you only need your CIIS/MyMail email address. 

Do I need to pay anything? 

No - this service is free so ignore any prompts about payment. 

Do I need a medical professional to sign anything? 

No - your vaccine card or other proof of vaccination is the medical verification - that's all you need to upload. 

When will I see my verification on Med + Proctor page? 

It may take approximately 72 hours for your verification to show up.  

Do I need to be able to scan and send a photo or PDF? 

Yes. 

I've been asked to re-upload my vaccine info, what do I do?

This is typically due to a corrupt or password protected file being uploaded. Navigate to login to Med + Proctor using your existing credentials. Once you are logged in, you can just re-upload your non-corrupt or non-password protected file and Med+Proctor will use your most recent upload to verify your vaccination.
